Q: What do I need to bring with me for the pre-employment health screening?

A: Should you be offered employment at Holy Cross, you will need to print and complete the Pre-Employment Health Screening Questionnaire. Click here to download.

It would also be helpful to bring the following information:

  • Last two tuberculin skin test results (formerly known as PPD tests)
  • Recent chest x-ray if you have a positive tuberculin skin test history
  • Lab results for Measles, Mumps, Rubella (MMR), Varicella, Hepatitis B

You will be required to provide a urine sample for drug screening on the day of your pre-employment screening. It is essential that you limit your fluids on the day of your pre-employment screening. A lab result determined to be diluted by our independent laboratory will prevent you from passing your pre-employment screening.

Q: Does Holy Cross Hospital, Inc. conduct a background investigation prior to employment?

A: Yes, all offers of employment are contingent on successful completion of the pre-employment health screening, drug screening and background investigation, inclusive of employment verification, work references, educational verification and criminal conviction information. Offers of employment may be rescinded for falsification of your application.

Q: What do I need to bring with me for payroll and benefit purposes?

A: Once you have cleared the background investigation, health screening and drug screening components of the pre-employment process, you will be scheduled to complete your new hire paperwork. For your convenience, we have included links to two of the forms you will be required to complete, your I-9 and your W-4. You should print and complete these forms and have the necessary forms of identification with you when you arrive for your appointment. If you do not have access to a printer, these forms are available in the Employment Office. If you are being hired for a position that requires licensure and/or certification, you must bring an original, unexpired document that attests to current licensure or certification. To attend orientation, you must complete your new hire paperwork by noon on Wednesday prior to your scheduled orientation date.

For payroll purposes, we must see your original Social Security card. If you do not have an original social security card, you will need to contact a local social security administration branch office and apply for a replacement card. There is no fee for a replacement card, but you must apply in person and you will be issued an official receipt the same day that you apply. This official receipt will suffice until your original card arrives in the mail.

Q: What about parking?

A: Holy Cross provides free parking in designated areas for Associates. To park in a designated area, you must obtain a parking permit. You will need to present your vehicle registration card to Protective Services personnel when your photo ID is made to obtain a parking permit.
